If you’re struggling with addiction and substance abuse, you have choices. There are a number of treatment and recovery options suited to different needs, including the 12-Step Program and SMART Recovery. For First Responders, either may work well, but one may be a better fit than the other.

The 12-Step Program, developed by Alcoholics Anonymous, is one of the best-known paths to recovery, but it’s not for everyone.[1] SMART Recovery offers an alternative approach that focuses on the disease model of addiction and self-empowerment.[2] It’s important to understand the differences between these two treatment options to determine the best choice for your needs.

What Is the 12-Step Program?

Created by the founders of Alcoholics Anonymous, the 12 Steps establish guidelines to overcome addiction to alcohol. After success in its early years, the 12 Steps were adapted to address other types of addiction, including opiates and other types of drugs.

Though the 12 Steps are based on spiritual principles, the program isn’t limited to religious people. The presence of God is prevalent, but with the understanding that people may interpret God differently.

The foundation of the 12 Steps provides support, encouragement, and accountability for people to overcome their addiction.

What Is SMART Recovery?

SMART Recovery stands for Self-Management and Recovery Training. This is a newer approach and an alternative to the 12-Step Program of Alcoholics Anonymous.

SMART Recovery focuses on cognitive-behavioral techniques and self-empowerment using scientific principles rather than spiritual principles. This is intended to solve the underlying issues that contribute to addiction.

SMART Recovery emphasizes four points:[3]

  • Building motivation
  • Problem-solving
  • Coping with urges
  • Balancing lifestyle

The SMART Recovery approach is tailored to the individuals experiencing substance abuse and addiction and appreciates that they can take charge of their addiction and behavior with the right tools and strategies.

12 Steps vs. SMART Recovery: Key Differences

The 12 Steps and SMART Recovery have some key differences.

The 12 Steps focus on powerlessness and acceptance, while SMART Recovery focuses on self-empowerment and ownership. The 12 Steps are also spiritual- or religion-based compared to the evidence-based approach of SMART Recovery.

Though 12-Step programs can vary, most require abstinence and use some iteration of the original 12 Steps to admit addiction and seek help from a higher power. SMART Recovery is more flexible and based on the individual’s goals, though it uses the four-point program as guidance.

What these programs share is a community or support system. The 12 Steps use sponsors and chips to denote milestones in the recovery journey, but these aren’t typically part of the SMART approach.

Which Program Is Right for Me?

With first responders, the inclusive community, stigma, and sense of willpower can be obstacles to getting help. Those struggling with addiction may fear that their colleagues and peers will judge them or that they may lose their jobs.

They may try to “go it alone,” drawing on the strength and willpower that are essential to performing their job duties. Only, addiction isn’t something that can be overcome with sheer willpower.

Both the 12 Steps and the SMART Recovery approach can be beneficial for first responders. Both programs focus on community and peer support – something that first responders know well. Otherwise, the best treatment option depends on the individual and their goals, and some may benefit from a combination of the two programs.
